RESIDENCY FORMS

Statement of Residency Form	If your family lives with someone that lives in the District you will need to fill out this form and turn it in to the appropriate school campus the child would attend. This form requires a Notary. A Notary is available on each campus.
Child Living Separate and Apart Form	If you have a child that will NOT be living with you, but will live with someone else in the District, you will need to fill out this form and turn it in to the appropriate school campus the child would attend. This form requires a Notary. A Notary is available on each campus.
